If you're returning home, make sure you have everything you need for your recovery. It may be helpful to ask a friend or relative to stay with you or visit regularly. If this is not possible, make sure you have plenty of food, drink and other essentials at home.

Staying physically and mentally well
The Healthy at Home service is a one-stop shop where you can access a range of online lifestyles services (e.g. stopping smoking and getting more active) that can keep you healthy during this time and beyond, completely free. Kingston Carers Network (KCN) can help if you are looking after someone returning from a hospital stay. KCN can help you with advice and advocacy, social activities and social groups.

Princess Alice Hospice Wellbeing Centre supports people with life-limiting illnesses and their carers to live as fully and independently as possible.

Need additional care support?
If you have been admitted to hospital and you, the staff, or anyone who supports you feels that you may have difficulty managing at home, hospital staff can ask for your needs to be assessed before you are discharged.
Practical help at home may include help getting in and out of bed, washing and dressing.
The assessment will evaluate your needs with you and the support available to meet them, depending on your situation.
If it's appropriate, we may arrange services to support you when you return home (called transitional care) depending on your needs and eligibility. Please call the council on 020 8547 5000 if you would like to talk through your options.

Our Elders Empowerment Programme aims to help older people in the community maintain their wellbeing. Our members are predominately Tamils but we welcome people from all backgrounds. We are running exercise sessions over Zoom, including yoga, dance and Tai Chi
