Community Connectors are trained Social Prescribing Link Workers aligned with local GPs. They work in partnership with Staywell, a local charity. They help people make positive changes in their lives. They do this by linking them to volunteers, activities and community groups who provide support.

Raleigh House day centre offers a warm and friendly social environment for people over 65 with activities, refreshments and lunch provided. We are able to support those living with a mild to moderate dementia or needing personal care services.
