You may feel that it is challenging to stay physically active when you have a medical condition or disability. We offer a low cost 12-week structured physical activity programme for adults with a medical condition, delivered by qualified exercise referral specialists.
Why Physical Activity?
There is a strong evidence base for regular physical activity in the treatment and prevention of the majority of diseases. Some of the benefits include:
Boosts wellbeing and mood
Strengthens and improves the function of muscles, bones and joints
Improves cardiovascular fitness
Contributes to maintaining a healthy weight
Helps to prevent or manage diabetes
Promotes sleep quality
Improves posture and balance

What we can offer you:
A 12 week structured exercise programme including 5 one-to-one sessions with an exercise specialist, ongoing support, motivation and advice.
A personalised training programme to suit your abilities, at your own pace
In-person gym sessions available at one of the participating local leisure centres*
*Each gym session or one-to-one session with the exercise specialist costs £4.20. A reduced rate gym membership is offered to those completing the full 12 week programme.

To qualify for this programme, you must:
Be 16 years old or over
Live, work or study in Kingston
Exercise less than 60 minutes a week
Have a Get Active referral form - your GP surgery will have access to this
You must also have have a recognised physical or mental health condition that may include one or more of the following:
At risk of coronary heart disease such as: family history, overweight, hypertension, type 2 diabetes
Mild depression, anxiety, stress and controlled mental health problems
At risk of developing or diagnosed with osteoporosis
Well controlled respiratory problems
Mild mobility problems e.g. arthritis, multiple sclerosis, joint problems
Motivated to give up tobacco or other substances
Controlled hypertension
Long COVID
Get Active also provide weekly exercise classes for people living with or at risk of diabetes at a rate of £4.20 per class. The NHS 'know your risk' tool can quickly help identify your personal risk for Type 2 diabetes.
