What is this service?

We support people with heart disease and people who have been told that they are at a high risk of developing cardiovascular disease.

We are made up of specialist nurses, a physiotherapist and cardiac exercise instructors.

We run clinics where we see people on an individual basis, and group programmes. We run low intensity programmes for people who are either higher risk or need a little more individual attention during exercise. This includes chair based exercise for those who need it.

We take referrals from secondary care, primary care, NHS Healthchecks and self referrals.

    We offer advice, information and guidance by phone/email for those caring for someone with mental health difficulties. Monthly Peer-support group. 1:1 Face-to-face sessions are available each month on wards and community service at Tolworth Hospital.
