Activity:
Your key worker will co produce a support plan in line with your objectives. You will agree what activities you would like to do with your key worker.
Purpose:
The purpose of this service is to support you in managing the challenges you face due to your mental health.
Requirement:
You must be a Kingston resident
Medium or low mental health issues
Your primary diagnosis must be mental health
