Our outreach service is here to support you with different parts of daily life where you may need some extra help.
Our friendly team will work with you to understand what support you need and match you with someone you feel comfortable with.
We can support you in a way that works for you, whether that’s at home, at work, in the community, or in hospital.
Support can range from a few hours a week to more regular help, depending on your situation. We can help you build confidence, develop everyday life skills, and work towards your goals.
We can also offer advice and support with things like:
managing money
keeping on top of your home
accessing other services
building positive relationships and support networks
We provide tailored support available to adults living in the boroughs of Kingston, Richmond, Merton, Wandsworth and the county of Surrey
