What is this service?

We provide support, advice and education for people with diabetes.

The service is led by a Community Diabetes Specialist Nurse whose main focus is the DESMOND programme, but she may visit patients at home or in a GP surgery (mainly to support GPs and Community or District Nurses).

Services delivered are:

  • Desmond – newly Diagnosed and Foundation
  • Clinical Advice and support for Practice Nurses and GPs
  • Support for Community Nurses
  • Cedars’ Unit, Tolworth Hospital support
  • Diabetes Education for staff
  • Walking Away from Diabetes - NEW PROGRAMME

DESMOND is a nationwide structured group education programme developed by experienced health care professionals in participation with people who have diabetes. The programme is for those newly diagnosed with Type 2 Diabetes. It is delivered by two accredited healthcare professionals trained to deliver the course and aims to support the person with diabetes, to become the expert.

At DESMOND you can:

  • Talk about your concerns
  • Get answers to your questions
  • Discuss the options for looking after your diabetes
  • Be supported in making your own choices about how you deal with your diabetes
  • Find out about what support you can expect from your local health service

Walking Away from Diabetes is a new three-hour education programme targeting people with either impaired glucose intolerance (IGT) or those at risk of diabetes as determined by the Leicester diabetes score developed in collaboration with Diabetes UK.
