What is this service?
The community (district) nursing service is a community based service providing skilled nursing care to adult patients over the age of 18 in their own homes, in residential homes or in a clinic setting. These patients are primarily housebound.
Community nursing supports and encourages people with a disability or long term condition to live independent lives, providing a range of services and treatment to enable individuals to avoid unnecessary hospital admissions and to facilitate early discharge where hospital admission has been necessary. Community Nurses also provide care to patients requiring palliative and end of life care.
