Community Neuro Rehabilitation

Your Healthcare provides specialist, multidisciplinary rehabilitation and disability management to adults with a neurological diagnosis who are aged over 18 years and registered with a Kingston GP.

The service aims to support and empower individuals and their carers to self manage their neurological conditions at whatever stage to help them do more things by themselves and have a better quality of life.

Our team provides supported discharge and community rehabilitation in a variety of settings where it is most appropriate. This can be in your own home, as an outpatient at Tolworth Hospital, or in any other appropriate community settings including gyms, day centres or place of work.

We work closely with you to provide a goal-orientated approach and rehabilitation package that meets your needs, with emphasis on achieving functional potential.

Treatment can be provided on a 1:1 basis or within a group depending on need.
